Children in entertainment: performance licence application
If you have a child at school who will be taking part in a performance, including television, theatre and photography, you must apply for an entertainment licence.
This is to make sure their education and welfare are protected.
How to apply
You must apply at least 21 days before the performance.
The performance licence application form is in two parts.
Part 1 should to be filled out by the television, theatre or production company.
Part 2 should be filled out by you. You must also provide the following documents:
- the child's birth certificate or other satisfactory evidence of the child's age
- two passport sized photographs of the child, taken during the six months before the application
- a medical certificate
- if your child will be absent from school for the performance you need authorisation from the headteacher

How the application is dealt with
We will check your application and send a copy of the licence to the television, theatre or production company and to you.
