Care at Home Service

We give priority to people with serious conditions or circumstances including:

• terminal, chronic or acute conditions or illness
• high risk to physical safety
• inability to carry out daily living tasks
• deterioration of living conditions where a person is at risk of infection
• to prevent a child or young person at risk being accommodated
• to avoid the need to go into a care home because the person's usual care(r) is not available
• breakdown in caring relationship
• support and rehabilitation after discharge from hospital
• vulnerable people preparing to live independently in the community
