Health and Care Partnership

The Health and Care Partnership was set up in 2002 to work with people and communities in South Lanarkshire to improve health and tackle health inequalities. As such it supports the Community Planning Partnership's aim to build safe and healthy communities and improve health and well being by aiming to reduce health inequalities.

The Health and Care Partnership has been overseeing the development of the Joint Health Improvement Plan - last updated in 2007-2008.

The Health and Care Partnership sets out its own complementary vision, aims and objectives and action plans in the Joint Health Improvement Plan (JHIP) for South Lanarkshire. The third JHIP, Stronger Together for Health 3, has been in place since April 2008.

The partnership also oversees the development of Joint Services. Like Joint Futures at national level, all the agencies involved in caring for people work together to ensure the services are well planned and co-ordinated across the area.
