Do not use this form to:
- apply for a crisis or community care grant
- submit evidence as part of an application that you have already made
- raise an appeal against a decision on your application.

Please note that we cannot advise on your eligibility to be awarded a crisis or community care grant without an application being submitted. If you have an enquiry or a fault relating to items you have received from the Community Care Grant, you must report this directly to The Furnishing Service on ccgenquiries@tfs.direct.
If you have already applied for a crisis or community care grant, please note our response timescales are:
- 2 working days for crisis grants
- 15 working days for community care grant
