This is an enquiry form only and not a confirmation of booking. If a registrar is available to conduct your ceremony your booking will be confirmed to you by our booking centre upon receipt of this form. You are responsible for booking your venue.
If you do not receive a response please phone 0303 123 1015 (select option 7).
If a registrar is not available on the proposed date our booking centre will advise you.
