Home Assessment Team (HAT)

We offer short‑term support in your home to help you regain skills, confidence and independence after an illness, disability or a change in your abilities. Our team works with you on the things that matter most to you, so you can do as much as possible for yourself and stay independent.

What we can help you with

We support you to build confidence and improve everyday tasks, such as:

  • preparing simple meals and drinks
  • washing, dressing and other personal care
  • finding new ways to manage tasks that have become difficult

Everything we do is focused on helping you achieve your own goals.

What to expect

A Social Work Assistant or Occupational Therapist will visit you at home to get to know your needs and agree goals with you. Together, we will create a short‑term support plan. Our Home Carers will then visit you to help you work towards these goals at a pace that suits you.

Reviews and what happens next

We will regularly review how you are managing. After each review, we will agree the next steps with you.

You may:

  • complete the service if you no longer need support
  • continue for a little longer if this would help
  • move on to ongoing care if you need more support

If you do need ongoing help, we will discuss the right option with you. This may be from our Care at Home service, or in some cases, from one of our trusted care providers.

Your home support diary

You will receive a home support diary. This includes your support plan, details of the staff visiting you and space for you to record your progress.
