Care at Home Service
Our Care at Home Service runs every day of the week and supports adults to stay as independent as possible in their own homes.
This often involves helping a person to wash, dress or get ready for bed. Home Carers are also trained to provide more personal care, such as helping with medication, managing incontinence, or looking after catheters.
They can also assist with everyday tasks and suggest easier ways to do daily activities. People who need a lot of support and don’t have anyone else to help them may have a Home Carer visit several times a day.
We also provide support at home with meals. You can request an assessment to see if you qualify for the Telecare/Community Alarm service.
You can watch our videos on Care at Home Services or supporting your independence.
Who can get care at home?
You can ask for care at home if you or someone you support is struggling at home because of:
- a serious or long‑term illness
- risk of harm or unsafe living conditions
- difficulty managing daily tasks
- a sudden change in health
- support needed after leaving the hospital
- a caring arrangement breaking down
- preparing to live independently
- a child or young person is at risk if support is not provided
We prioritise people with the greatest risks or needs.
How to request care at home
You can ask for care at home after a social work assessment. At this meeting, you, your family, carers and health or social work staff talk about what support will help you. If care is agreed, you will receive a personal support plan and a home care diary that records how things are going.
Request support by completing our social work enquiry form or phoning 0303 123 1008 during office hours.
Costs
Some people pay for part of their care at home. We explain any charges before you agree to the service. You can pay online or in person at a PayPoint or Post Office.
