| Conclusion | The Joint Community Care Plan is prepared on behalf of the South Lanarkshire Health and Care Partenership. It sets out the health and social care priorities which we will concentrate on over the next 2 years and the outcomes which we are commited to achieving. Tackling inequalities is central to the social care and well being agenda. The initial impact assessment has not identified any adverse effects on target equality groups and as such no further work is required. |
