Continuing care and aftercare

Local Social Work offices provide throughcare and aftercare services to young people approaching school leaving age that have been looked after and accommodated by South Lanarkshire Council.

Young people looked after at home can also be given advice, guidance or support and this can be accessed through their social worker.

Young people who are looked after and accommodated in children's houses, foster care, residential schools, external placements or secure units may need support and advice. The social worker will work with the young person and begin the Pathways Assessment and planning process. The focus will be on preparing the young person to develop independent living skills which will help them to make a successful transition to independent living. This help is available to young people until they reach the age of 21 years.

Close links have been established with residential staff, housing, health, education and other services which support young people. Work is also carried out with the young person's family/carers.
