Continuing care and aftercare support
Support for young people in care or leaving care
We support young people in South Lanarkshire who are in care or leaving care. If you are aged 16 or over and live in foster care or residential care, you can stay in your home until you are 21. If you have left care, we can support you until you turn 26.
Our team is based at the Jimmy Swinburne Centre in Blantyre, where you can get practical help, emotional support and a safe space to talk about your plans for the future.
Who we support
You can get support from us if you are:
- aged 16 to 21 and want to stay in your foster or residential home
- a young person who has left care and is under 26
- an unaccompanied asylum‑seeking young person who has arrived locally or through the National Transfer Scheme
How we can help
We work alongside you to build strong, trusting relationships so you feel supported as you move into adulthood. We can help you with:
- finding or keeping a home
- managing money
- education, training or work
- physical and mental wellbeing
- staying connected with people who matter to you
Everything we do is based on what you tell us you need. We aim to make your move into adult life planned, supported and informed.
Visit our community hub
You can meet us and our partners at our community hub in Blantyre. It is a relaxed, welcoming place where you can get support, take part in activities and connect with people who can help you.
