Care and support for adults with learning disabilities

We support adults with learning disabilities and complex needs to live safely and independently in their own homes. Our team works with you to build confidence, stay well and remain an active part of your community.

You can get help with day‑to‑day tasks, managing your home and taking part in activities that matter to you. Support is based on your needs and what will help you live the life you want.

What we can help you with

We offer practical, personal and social support, including:

• personal care such as washing and dressing
• support with household tasks
• help to manage your tenancy and keep your home safe
• support to budget, pay bills and manage your money
• help to organise and take medication
• support to get involved in community groups, hobbies and social activities

Your support plan will be created with you, and we will review it regularly to make sure it continues to meet your needs.

Charges and financial assessments

Some services may have a charge. Your social worker will carry out a financial assessment every year to check if you need to pay anything and to make sure it is fair and up to date.
