Annual HAVS screening assessment

The health surveillance program aims to protect workers' health and assess the effectiveness of vibration control measures. Its goal is to prevent disabling hand function loss from Hand-Arm Syndrome (HAVS). Surveillance is required if a risk assessment identifies a need and runs alongside a programme of vibration control measures.

Employees working with vibrating equipment, who are identified as at risk for HAVS, Vibration White Finger (VWF), or whole body vibration must complete this questionnaire annually. Your line manager will notify you when to complete and submit the form, which is available below, for direct submission to the Occupational Nurse.
