The State Hospital Service Social Work

Accessing the service

The Social Work Service contributes to the pre-admission assessment process within The State Hospital and, where possible, will meet with a patient prior to admission to contribute to the assessment, care and treatment planning processes.

Each patient is allocated a Social Worker on admission and will continue to receive a Social Work Service for the duration of their time within the hospital.

The allocated Social Worker will, where appropriate, be in contact with the patient's family and/or carer and seek to support contact and promote positive social networks.

Social Work continue to provide a service to patients until the point of discharge and will liaise with relevant partner agencies at this stage in the patient’s journey.
