Adult and older people services
We support adults and older people to live safely, independently and with dignity. Our adult and older people services are one joined‑up service, so you get the right help at the right time, close to where you live. You can contact us if you need care yourself, support for someone you care for, or advice about what help is available.
How we can help
We work with health services, housing, and local voluntary groups to provide support in the community. Our staff work from local social work offices, day support centres, community settings and people’s own homes. Some workers are also part of joint teams with health and housing staff.
Assessments and care planning
If you need support, we will talk with you and, if you want, your carer or family. We assess your needs and agree on a plan that focuses on what matters to you and how you want to live your life. This may include support at home, day support, short‑term help, or longer‑term care.
Support at home and in the community
Home care and day care services help you stay as independent as possible. This can include personal care, help with daily tasks, social activities and support to stay connected in your community.
Care homes
Care home support is available if you need care in a residential setting. This can be for a short stay, for example, after time in hospital, or for longer‑term care if living at home is no longer right for you.
Our legal duties
Some of our work is guided by law, including mental health and adults with incapacity legislation. This helps make sure people’s rights, safety and wellbeing are protected.
